2025-2026 Enrollment Documents

Enrollment documents are fillable PDF documents. These documents may be downloaded to your home computer, then electronically manipulated to add information and print, or you may print the document and handwrite the information.

In-person enrollment is July 30, 2025, at 12:00–8:00 PM in the Udall High School Commons and Gymnasium located at the corner of Fourth Street and Seymour Street. Enrollment forms will be available for in-person enrollment.

Please complete the following documents per student to enroll in USD 463 for the 2025-2026 school year:
